Adult Day Care Admissions

The first step in the admission process, once you’ve had an initial visit, is to fill out our application form and email it to ev********@**********er.org. Visitors will have a chance to walk the halls and get a true feel for the experience. We encourage everyone to ask any unanswered questions at any point along the way. Choosing a day program for yourself or a loved one can be daunting, but the staff at Vassar-Warner Home is here to help and make the transition as smooth as possible.

Eligibility: To be eligible for our Social Adult Day Care program, seniors must demonstrate the need for assistance with one or more activities of daily living or require supervision due to a mental impairment.

Payment Options: Daily and half-day rates are available. We accept private pay and Medicaid Managed Long Term Care (MLTC) plans. Resources may be available to help offset costs depending on eligibility.
